Bajamunde, Jr.) "A RESOLUTION AUTHORIZING THE HONORABLE CITY MAYOR TO RENEW AND SIGN, FOR AND IN BEHALF OF THE CITY GOVERNMENT OF PAGADIAN, THE CONTRACT OF SERVICES WITH DR. ALMERA M. HOFILENA, M.D., AS RADIOLOGIST AT THE PAGADIAN CITY INFIRMARY CLINICAL LABORATORY FOR A SPECIFIC PERIOD FROM AUGUST 01, 2023 UP TO DECEMBER 31, 2023" WHEREAS, in the pursuit of its mandate to provide better health services to the city's constituents, the City Government of Pagadian has established a Clinical Laboratory that is operated at the Pagadian City Infirmary, and as the maintenance and operation of such laboratory requires the services of a radiologist, a field of specialization that is not currently available in the present personnel structure in the City Government, the services and expertise of Dr. Almera M. Hofilena, M.D., a licensed medical practitioner specializing in Radiology, had been engaged through a Contract of Services; WHEREAS, the Honorable City Mayor, in view of the expiration of the Contract of Services previously entered into by and between the City Government and Dr. Almera M. Hofilena, M.D. and considering the need to further extend such services for a specific period from August 01, 2023 up to December 31, 2023, sought for authority to renew the said contract with a monthly honorarium of Ten Thousand (P10,000.00) Pesos plus One Hundred (P100.00) Pesos per reading; WHEREAS, this body finds the request of the City Mayor meritorious and worthy of support hence granting him the required authority to enter into such contract of services is imperative; NOW, THEREFORE, on seconded motion of Honorable Jesus B. Bajamunde, Jr., be it RESOLVED to authorize, as this body hereby authorizes the Honorable City Mayor to renew and sign, for and in behalf of the City Government of Pagadian, the Contract of Services with Dr. Almera M. Hofilena, M.D., as Radiologist at the Pagadian City Infirmary Clinical Laboratory for a specific period from August 01, 2023 up to December 31, 2023. RESOLVED, FURTHER, to let copies of this resolution be forwarded to His Honor, City Mayor Samuel S. Co; to the City Legal Officer; to Dr. Debbylene T. Ildefonso, M.D., City Health Officer; and to Dr. Almera M. Hofilena, M.D., all of this city, for information, guidance and/or appropriate action. APPROVED UNANIMOUSLY.
