"A RESOLUTION AUTHORIZING THE HONORABLE CITY MAYOR TO ENTER, FOR AND IN BEHALF OF THE CITY GOVERNMENT OF PAGADIAN, INTO A MEMORANDUM OF AGREEMENT WITH THE DEPARTMENT OF HEALTH--CENTER FOR HEALTH DEVELOPMENT ZAMBOANGA PENINSULA, IN RELATION TO THE PROVISION AND RELEASE OF MOBILIZATION ALLOWANCE TO HEALTH WORKERS AND CASH GRANTS OF TWO HUNDRED THOUSAND (P200,000.00) PESOS FOR BEING TOP PERFORMING LOCAL GOVERNMENT UNIT DURING THE MEASLES-RUBELLA AND ORAL POLIO SUPPLEMENTAL IMMUNIZATION ACTIVITY" 
 
WHEREAS, the Department of Health Zamboanga Peninsula-Center for Health Development, on 17 November 2020, issued Department Memorandum No. 134, s. 2020, which, in gist, established the guidelines for the provision of mobilization allowance to health workers and cash grants to top performing local government units during the Measles-Rubella and Oral Polio Supplemental Immunization Activity;

WHEREAS, based on the foregoing guidelines, Pagadian City was adjudged eligible to for a cash grant in the amount of P200,000.00;

WHEREAS, to hasten the release of such grant/incentives, the formal forging of a Memorandum of Agreement by and between the Department of Health and the City Government of Pagadian-being the recipient, such that the Honorable City Mayor sought for authority to enter into the said deed for and in behalf of the City Government, which request merited the support and concurrence of this august body; 

NOW, THEREFORE, on seconded motion of Honorable Teodoro M. Quicoy, be it

RESOLVED to authorize, as this body hereby authorizes the Honorable City Mayor to enter, for and in behalf of the City Government of Pagadian, into a Memorandum of Agreement with the Department of Health-Center for Health Development Zamboanga Peninsula, in relation to the provision and release of mobilization allowance to health workers and cash grants of Two Hundred Thousand (P200,000.00) Pesos for being top performing local government unit during the Measles-Rubella and Oral Polio Supplemental Immunization Activity. 

RESOLVED FURTHER, to let copies of this Resolution be furnished His Honor, City Mayor Samuel S. Co, for his immediate favorable consideration; to the City Legal Officer; and the Department of Health, Regional Office 9, through Emilia P. Monicimpo, MD, MPH, CSEE, Director IV, Department of Health-RO 9, and all concerned for their information and guidance.
