Fernandez) "A RESOLUTION AUTHORIZING THE HONORABLE CITY MAYOR TO RENEW AND SIGN, FOR AND IN BEHALF OF THE CITY GOVERNMENT OF PAGADIAN, THE CONTRACT OF SERVICES WITH DR. ESTHER IRENE C. LACSON, M.D., FOR A SPECIFIC PERIOD FROM JANUARY 01, 2020 TO DECEMBER 31, 2020, WITH REFERENCE TO THE IMPLEMENTATION OF TB-DOTS PROGRAM IN THE CITY OF PAGADIAN" WHEREAS, the City Government of Pagadian, consistent with its thrust to further advance its delivery of basic health services more particularly in providing assistance to the Medical Officers of the City Health Office in the implementation of the TB-DOTS Program, had engaged the services and expertise of Dr. Esther Irene C. Lacson-a licensed medical practitioner and an Internist-through a Contract of Services; WHEREAS, the Honorable City Mayor-in view of the expiration of the Contract of Services previously entered into by and between the City Government and Dr. Esther Irene C. Lacson and considering the need to further extend such services-sought for authority to renew the said contract for a specific period from January 01, 2020 to December 31, 2020, with a monthly professional fee of Two Thousand Two Hundred Fifty (P2,250.00) Pesos; WHEREAS, this body finds the request of the City Mayor meritorious and worthy of support hence granting him the required authority to enter into such contract of services is imperative; NOW, THEREFORE, on seconded motion of Honorable Priscilla M. Fernandez, be it RESOLVED to authorize, as this body hereby authorizes the Honorable City Mayor to renew and sign, for and in behalf of the City Government of Pagadian, the Contract of Services with Dr. Esther Irene C. Lacson, M.D., for a specific period from January 01, 2020 to December 31, 2020, with reference to the implementation of TB-DOTS program in the City of Pagadian. RESOLVED FURTHER, to let copies of this resolution be forwarded to His Honor, City Mayor Samuel S. Co; the City Legal Officer; and Dr. Esther Irene C.
