RESOLUTION NO. 2019-0201 (Sponsor: Honorable Priscilla M. Fernandez) "A RESOLUTION AUTHORIZING HONORABLE CITY MAYOR SAMUEL S. CO TO ENTER, FOR AND IN BEHALF OF THE CITY GOVERNMENT OF PAGADIAN, INTO A CONTRACT OF SERVICE WITH DR. ESTHER IRENE C. LACSON, M.D., FOR A SPECIFIC PERIOD FROM JULY 1, 2019 TO DECEMBER 31, 2019, WITH REFERENCE TO THE IMPLEMENTATION OF TB-DOTS PROGRAM IN THE CITY OF PAGADIAN" WHEREAS, the City Government through the Pagadian City Health Office, in line with its thrust and mandate to provide better health services to the city's constituents and in support of the national government program, is pursuing the implementation of Tuberculosis-Directly Observed Treatment Short Course (TB-DOTS) Program in the City of Pagadian; WHEREAS, in pursuing such program, it requires the services of an internist-a field of specialization that is not currently available in the present structure of the City Government, thus the Honorable City Mayor sought the technical services of Dr. Esther Irene C. Lacson, a medical doctor by profession with specialization in the field of internal medicine, for a period from July 1, 2019 up to December 31, 2019, who has signified her willingness to make available her services for the purpose; WHEREAS, in order to effect such services, there is a need to execute a Contract of Service where the obligations and responsibilities of both parties are being specified, such that the City Chief Executive requested that he be accorded with the necessary authority to represent the City Government of Pagadian in the formalization of the service contract, which request merited the support and concurrence of this august body; NOW, THEREFORE, on seconded motion of Honorable Priscilla M. Fernandez, be it RESOLVED to authorize, as this body hereby authorizes the Honorable City Mayor to enter, for and in behalf of the City Government of Pagadian, into a Contract of Service with Dr. Esther Irene C. Lacson, M.D., for a specific period from July 1, 2019 to December 31, 2019, with reference to the implementation of TB-DOTS Program in the City of Pagadian. RESOLVED FURTHER, to let copies of this Resolution be forwarded to the Honorable City Mayor, the City Legal Officer, the City Budget Officer, the City Accountant, the City Administrator, the City Auditor and Dr. Esther Irene C.
